Job Title: Care Manager - FT

 

Position Scope: The Care Manager is responsible for providing personal assistance and routine daily care and services related to the residents in accordance with the Resident Care Plan.

 Reports to: Director of Health & Wellness

 

FLSA Status: Full-Time, Non-Exempt

RESPONSIBILITIES: • Assists residents in their Activities of Daily Living (ADL) tasks as needed and directed. This could include bathing, grooming, personal and dental hygiene, transportation to meals and activities, bowel and bladder functions (e.g., take to bathroom, portable commode, change incontinency products, etc.), shaving, dressing, hair care, and laundry tasks. All tasks and duties as described or documented in the resident’s service plan. • Provides resident comfort by utilizing resources and materials; transporting residents; answering resident’s call lights in a timely manner; meeting the expectation of five (5) minutes or less, answering resident’s requests; reporting observations of the resident to nursing supervisor. • Responds to resident’s needs promptly while maintaining each resident’s self- respect, personal dignity, personal safety and confidentiality. • Assists residents into and out of bed and wheelchair as needed, following safe lifting/transferring techniques. • Assures that residents are dressed and groomed appropriately. • Assists Engagement staff in carrying out planned activities. • Documents actions by completing forms, reports, logs and records electronically. • Uses Universal Precautions and follows established policies concerning exposure to blood/body fluids. • Serves and protects the community by adhering to professional standards, community policies and procedures, state and local requirements. • Updates job knowledge by participating in education opportunities; reading professional publications; participating in professional organizations and maintaining certification/licensure. • Maintains a safe and secure environment for all staff, residents and guests, following established safety standards. • Adheres to all mandated reporting requirements and communicates concerns to the person in charge.

QUALIFICATIONS: • Ability to work under pressure and relate well to residents • CPR certification a plus • Excellent bedside manner • Strong organizational skills and attention to detail • Excellent written and verbal communication skills • Experience working with an electronic health record system

 

PHYSICAL QUALIFICATIONS: • Walk/Stand – must be able to continuously walking and standing • Environment Condition – must be able to perform work both inside and outside • Sit – sit infrequently • Lift - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds • Carry - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds • Push - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds • Climb – must be able to climb stairs on a frequent basis, with the opportunity to utilize the elevator as an alternate resource if available • Bend – must be able to bend at the waist, knees, hips and spine on a frequent basis while physically assisting a resident; may require frequently manipulating weights of 25 pounds or more. • Squat – must be able to squat on a frequent basis to reach below the knee to assist with physical care of a resident requiring lift of 50 pounds or less. • Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and neck while providing physical assistance for residents, manipulating weights of 25 pounds or more. • Reach– must be able to reach on a frequent basis while providing for resident care. May occasionally reach overhead, requiring manipulation of weight of 10 pounds or less, and below waist manipulating weight of 25 pounds or more. • Hand Dexterity - Constant requirement of hand dexterity for arranging medications and resident personal care items such as dentures, hearing aids and other grooming items. Occasional typing, telephone usage and paperwork sorting • Sensory Vision – must be able to read clearly with or without corrective lenses • Hear - Must be able to hear telephone, audible alarms, bells and signals related to resident safety with or without hearing devices. • Speech/Language – must have strong command of English sufficient to read and write and interpret medical and administration information.
