Social Support Facilitator

Cheshire Ireland provides quality services to adults with physical,sensory and neurological disabilities at a medium and high level of need. We provide individualised supports to promote wellbeing, independence and quality of life for all people who use our services. We currently provide services to over 200 people in their own homes, in residential centres, in supported accommodation and in respite facilities. Established in Shillelagh, Co.Wicklow in 1963, Cheshire Ireland are a Section 39 organisation funded by the HSE. We employ over 800 staff and operate in Dublin, Cork, Galway, Wicklow, Limerick, Donegal, Kerry, Carlow, Waterford, Sligo and Mayo.

The people we are looking to recruit will be 10 hours Permanent, in Galway Cheshire services . This service is based Roscam Galway.



What we do?

•The purpose of this role is to support an individual to build connections in the community and to assist them to access social outlets/events which are meaningful to them.

•To work in a targeted and focused manner to ensure that individuals are facilitated and empowered to make contact with others which they share common interests.

•To facilitate individuals to be proactive in managing and directing their choices. 

•It is critical when undertaking your work that you do so in a manner that is consistent with Cheshire Ireland’s values and ethos.

The Cheshire Community Living Service is based on clear principles of supporting people “one person at a time”. As a Cheshire Social Supports Facilitator you will be committed to the provision of quality services and to the concept of working in partnership with clients who live in their own homes.

What you will do.

The successful candidates will have the ability to operate independently with good use of initiative, and also to work effectively in a small team of staff. You and your team will provide a level of personal care in line with client health needs, exercise programmes, and household management. You and your team will support clients’ safety, security, emotional, and spiritual well-being whilst also supporting people, if necessary, to identify their individual goals and achieve future aspirations.
