**This is a full-time position based out of our Ozark Corporate office**

Internal Title: Patient Care Coordinator

Reports to: Clinical Director

Responsibilities of the RN Clinical Manager include but are not limited to:

● Reviews and approves OASIS

● Ensures that patient needs are continually assessed and care rendered is individualized

to patient needs, is appropriate and reasonable, meets home health eligibility criteria,

and is in accordance with physician orders

● Oversees and assures development, implementation, and updates to the individualized

patient plan of care as appropriate

● Reviews assessments and plans of care daily per the assigned workflow, and consults

clinicians with recommendations as appropriate

● Works closely with the Executive team to develop and implement further activities

designed to improve patient outcomes and clinical documentation

● Provides direct patient care, as necessary, in accordance to scope of practice and

physician orders

● Supports and upholds all federal state, and local regulatory laws and standards

● Supports and upholds Haven Home Health policies and procedures

● All other duties as assigned

Qualifications, Knowledge, Skills, and Abilities

● Current Missouri Registered Nurse license in good standing

● Minimum of one year experience as an RN within the last three years

● Experience processing OASIS in the field

● Excellent organizational and time management skills

● Excellent communication skills with multidisciplinary team of clinicians, clerical staff, and


● Flexibility in work schedule to accommodate patient needs, including new admissions

● Strong clinical judgment, assessment, communication, and leadership skills

● Proficient in computer skills and technology to complete documentation of clinical record

● Must have dependable transportation and a current driver’s license

● Will work weekends and holidays as needed

Preferred Qualifications:


● Recent home health experience in the past year

● Certification in a specialty area by a national organization such as American Nurses

Credentialing Center (ANCC)
